Assisted Accommodation with Housing Related Support and Tenancy Support Services

Overview

Lincolnshire Partnership NHS Foundation Trust recognises the significant role that suitable accommodation, housing stability and social inclusion play in supporting individuals to live independently, participate within their communities and maintain sustainable tenancies. Access to safe, affordable and appropriate accommodation is a key factor in improving wellbeing, reducing social exclusion and supporting positive life outcomes. Housing instability is frequently associated with increased reliance on statutory services, tenancy breakdown and barriers to independent living. The service is required for eligible adults leaving LPFT inpatient and rehabilitation services who need accommodation-transition and tenancy support. It will be used by Service Users, authorised LPFT referral teams and partner agencies involved in discharge, housing, welfare, health and social care. The required model will maintain the core accommodation-sourcing, discharge and tenancy-sustainment service while strengthening measurable outcomes, tenancy sustainment at 3, 6 and 12 months, Service User experience, accommodation-sourcing timescales, void management, equality analysis and value-for-money assurance. The current service is an established provision. Commissioned capacity is 15 active concurrent Service Users; recent performance information reports average occupancy of 93%, 27 referrals since April 2024, a 74% acceptance rate and positive move-on outcomes for all recorded Service Users. The service supports broader integrated system objectives by improving access to housing options, tenancy sustainment services and coordinated support arrangements across Lincolnshire. It also supports national and local priorities to move support from hospital to community, prevent avoidable delay, improve independence, address housing as a determinant of health and enable people to live well in their communities.
