Mae'r gwasanaeth hefyd yn cyfrannu at Ddeddf Llesiant Cenedlaethau'r Dyfodol (Cymru) 2015 drwy gefnogi atal, integreiddio, cydweithio a chynaliadwyedd hirdymor gwasanaethau. Gall y gwaith a gyflawnir amrywio a chynnwys (nid rhestr gynhwysfawr yw hon): - Rheiliau llaw (maint a deunyddiau amrywiol) - Rheiliau grisiau (maint a deunyddiau amrywiol) - Camau drws - Blychau diogel ar gyfer allweddi - Addasiadau i ystafelloedd ymolchi The Minor Adaptations Partnership (MAP) contributes to the principles of the Social Services and Well-being (Wales) Act 2014 by promoting wellbeing, prevention, co-production, voice and control, and early intervention. It supports individuals to remain safely in their own homes and communities, reducing the need for escalating health and care interventions. The project also contributes to the Well-being of Future Generations (Wales) Act 2015 by supporting prevention, integration, collaboration and long-term sustainability of services. The work completed can vary and include (not an exhaustive list): - Handrails (various sizes and materials) - Stair Rails (various sizes and materials) - Doorsteps - Key Safes - Bathroom adaptations
